Provisional Custody By Mandate
In Louisiana, we have a document that authorizes another person temporary parental authority to do things like enroll them in school and take them to the doctor. In the recent Hurricane Ida aftermath, many parents sent their children to friends and relatives until their homes were complete. This form allows them to act on your behalf for up to a period of one year.
Travel Affidavit
Is your child going to visit relatives with someone other than a parent? You may need a travel affidavit, which is a document that describes the details of the child's travel arrangements and who is traveling with them.
